Multicultural approach in Pre-hospital care

The topic of the bachelor thesis is dedicated to multicultural nursing specifically in the conditions of pre-hospital emergency care. In today's globalized society it is necessary for paramedics to have basic knowledge of multicultural care. The theoretical part defines basic concepts in multicultural treatment, it is dedicated to communication and characterizes individual minorities living in the Czech Republic. For the practical part, there were three goals set: 1. To map out the approach of paramedics while providing care to a member of another nationality in the conditions of pre-hospital emergency care. 2. To map out the experience of paramedics with the individuals of given cultures within pre-hospital emergency care. 3. Find out the opinion of paramedics on whether they were sufficiently prepared for multicultural care within the qualification study. To fulfil set goals, qualitative research method was used, via the approach of queries and semi-structured interview technique. The research sample consisted of paramedics of the Karlovy Vary and South Bohemia regions with various lengths of experience. The research investigation found, that paramedics choose the same approach when in contact with ethnic groups. They are aware of various differences, though the most common problem is the communication barrier. The paramedics have both positive and negative experiences considering providing care to minorities. The research revealed, that the education about multicultural care is mostly insufficient and that the paramedics are willing to further educate themselves on the issue. Based on the research, an option of developing a special application was proposed. It would be able to quickly and precisely translate even professional medical terms from different languages in action. An information brochure about the principles of providing pre-hospital emergency care to ethnic groups was also made. Furthermore, it would also be possible to compile information from the bachelor thesis into scripts, which would be intended for teaching about multicultural nursing.
Foreign language as a communication barrier in intensive care

This diploma thesis analyzes if there is communication barrier with foreign patients at the selected intensive care units. We dealt with question whether hospital employees perceive communication with foreigners as a problem or not and how can they solve the problem. The first (theoretical) part is devoted to the role of communication in nursing process and to the definition of term communication barrier. In next chapters we have analyzed the profile of multicultural nursing. Then we focus on the issues of cross-cultural communication and communication barriers in contact with foreigners and the most common causes and techniques used to deal with such situations. We pay great attention to prevention and potential solutions to barriers between cultures and ethnic groups. The motivation for the thesis was supposed effecto of communication gaps on health and higher psychological burden on staff, especially when one of the basic activities of nurses is to educate patients. Research interprets the information obtained from a self-designed questionnaire survey. Questionnaires were distributed to intensive care and resuscitation units in four hospitals in Prague. Transcultural Midwifery Care Focusing on the Mongolian Women Living in the Czech Republic

The dissertation aims at the issue of transcultural care in the practice of midwives with a specific focus on Mongolian women, who currently represent the fifth largest group in terms of the number of births of foreigners in the Czech Republic. The theoretical framework of the text consists of chapters on nursing with subsequent differentiation of the field with regard to specialized care in the field of gynecology-obstetrics, as well as the divided subdiscipline of transcultural nursing care. An integral part of the theoretical part of the work are the basic general information about Mongolia and the Mongols. The practical part of the work is represented by my own qualitative and quantitative research. Qualitative research was carried out in the field through in-depth interviews with Mongolian women living in the Czech Republic and having experience with their own childbirth in a Czech maternity hospital. The aim of the research was to uncover possible peculiarities and requirements that a midwife may encounter from Mongolian women in the provision of transcultural care related to pregnancy, childbirth and the postnatal period. Communication cards for neonatal departments

The bachelor thesis deals with the production of educational material for neonatal wards in order to improve communication between medical staff and the individual who takes care of a newborn. The cards are described in four different languages to be useful not only for Czech-speaking residents of Czech Republic. The thesis uses qualitative methodology. The theoretical part is divided into four chapters that explain the concept of communication, discuss pictograms as a form of communication and multicultural nursing. It continues with basic care of the physiological newborn, while in the last chapter we deal with concepts from neonatology, such as Bonding and the rooming-in system. The work of a nurse on the abroad mission

Now a days nurses can work not only in health institutions but they can get involved in abroad missions. In the Czech Republic there are several organizations which offer humanitarian help in the countries of the third world. The concept of the bachelor thesis is theoretical - empiric. The theoretical part is devided into some parts. The first part is about globalization and about migration of people. Then there is written about religion and about individual humanitarian organizations. The other chapter points to the role of the nurse in the past and now a days. The last part of theoretical work contains demands that you must fulfil before your abroad mission. Then there is written about diseases which you can meet during treatment on the abroad mission. The second part of the bachelor thesis is a research survey. The aim of the work was to find what the nurse must do on the abroad mission in individual organizations. Three research questions have been identified to achieve the goal. What is the preparation of the nurse before leaving? What is the specific of her work on her abroad mission? What is the experience of nurses on the abroad mission? In the empiric part of the thesis a method of qualitative research surney. Responses were obtained through a pre-prepared semi-structured interview with nurses who took part in some abroad missions. From the researching results follows, that the main aim of the nurse on the abroad mission is to treat wounded persons during the war conflicts. But she must take care of expectant mothers and new born babies too. She looks after inhabitants who didnt't have enough money to pay their medical treatment in the hospital. She educates local health personnel to give a specialist health treatment to sick persons. The results can help students and nurses to know their possibilities to go to the abroad missions with the humanitarian organization.
Transcultural differences in the care for children´s patient

Due to the higher migration rates, there is more cultural diffusion. The healthe care personnel will be dealing with patients from different cultures more often. This is why it is important to use an appropriate approach and give competent heathcare considering cultural backround of the patients. The goal of this thesis was to map the cultural differences in care for an underage patient and to trend the minority groups that are being cared for by nurses in the children's ward. To achieve the goal of this thesis, there were four specified questions and to achieve the result, there was a threephase trial, specifically qualitative trial supplemented with quantitative method using a questionnaire. The first phase was the pilot poll and a techniquire "snowball slamping" was used. The second phase of the trial consisted of pre-structured interviews with the nurses. The third phase consisted of pre-structured interviews with the parents. On the basis of this trial, it was concluded that the nurses are caring for Romany and Vietnamese minority groups most often. The other most patiens from Ukraine, Russia, Germany, Poland, Muslims and Jehova witnesses. The most common problems that occur during the care are: language barrier, cultural customs, communication and co-operation. Based on the data collected from interviews with nurses and parents, there are specific variations in expectency of care, such as food, daily regime, differences in behavour, communication, reliability to adhere to given rules and consequent care after the hospital stay. There are other different specifics such as rituals and traditions during child labour or at the time of death, celebration of different festivals, fasting and other spiritual needs. Traditions relating to refusal or demand of specific care were also mentioned. Results of this thesis were presented at the 13th Student conference in Pardubice and will be published in a technical journal.
The issues of communication of pediatric nurses with the parents of the patients coming from the certain ethnic groups

The aim of this thesis was to find out barriers which occur during the communication of pediatric nurses with the parents of patients belonging to the other ethnic groups and how they deal with these problems. More specifically, it was ethnic Vietnamese, Romany and Ukrainian. The theoretical part deals with the concepts belonging to multicultural nursing, general principles of communication and its forms. It also described three ethnic groups living in the territory of the Czech Republic, together with the characteristics of their specifics in the field of culture, health and communication. The research part of the thesis was divided into three phases. The first phase consisted of interviews with nurses in GP surgery for children and adolescents and nurses working at the peditric department, as well as additional interviews with parents of child patients belonging to another ethnic group which recorded view at the communication from their side. The second phase of the research was concerned with observing the communication of nurses with representatives of ethnic groups in practice. There were three observations in three different GP surgeries for children and adolescents. During this part of the research, there were identified deficiencies that occur in communication. The third stage of the research was built on the basis of these findings. Its aim was to give child nurses recommendations that can make their communication more effective in practice. In conclusion, the nurses were acquainted with the information brochure, which contains information on how to communicate with foreigners. The research has shown that the greatest problem in communication with representatives of other ethnic groups is, according to pediatric nurses, the lack of knowledge of foreign languages, as well as the lack of knowledge about the cultural and communication specificities of the representatives of the Vietnamese, Romany and Ukrainian ethnic groups. It was also found that nurses are very interested in information on multicultural nursing.
Gipsy patients in Czech medical care

The theoretical part of the bachelor thesis is devoted to multicultural nursing, Roma ethnics, Romany history, Romany family, lifestyle and communication with patient from minority community. There are also topics such as pregnancy and childbirth, dying and death and, last but not least, Romany attitude towards health and disease. The practical part of the bachelor thesis is focused on Romany patients who have been hospitalized in various departments in the hospital and nurses from inpatient wards who provide nursing care during their hospitalization. For the research part of this bachelor thesis, a method of qualitative research was used, using semi-structured interviews with patients from minor community and also with nurses from the hospital. The aim of the thesis was to find out what are the specifics of nursing care for a Romany patient. One of the most important specifics of nursing care for a Romany patient is repeated education on ongoing treatment and nursing care and the need to place great emphasis on intelligible information during hospitalization. Another aim was to map out the most common problems encountered by Romany during hospitalization in the hospital, such as the feeling of limited freedom, separation from family and disturbed privacy. The third objective was to map out the problems encountered by nurses in nursing care of a Romany patient. Among the main problems mentioned, the nurses have encountered difficulties in adhering to the set rules of hospital facilities and insufficient or limited cooperation and communication with these patients. The obtained answers from the interviews of the interviewed nurses and Romany patients were classified into individual categories and subcategories. This bachelor thesis could raise awareness of this issue among healthcare professionals and improve the quality of nursing care for Romany patients during their hospitalization. The data and information obtained were summarized and processed in the information leaflet for nurses who treat patients from minor community.
Pain perception in selected ethnic and religious minorities

The diploma thesis is focused on the perception of pain across selected cultures and religions. The thesis is divided into theoretical and practical research. The theoretical part describes transcultural nursing and development, pain and perception. Subsequently, selected ethnic and religious minorities, namely the Vietnamese, Ukrainian, Roma minority and Islamic minority, are characterized. In the practical part, the data obtained during the research survey are processed. At the beginning of the research were compiled two goals. The first was aimed on mapping the perception of pain among members of selected national and religious minorities. The second objective was focused on the use of pain therapy options among members of selected national and religious minorities. Data were obtained using a semi-structured interview of 25 open questions. The interviews were attended by five probands from each minority residing in the territory of the Czech Republic. Only an Islamic minority was represented by two members residing in the territory of the Czech Republic. It can be seen from the results that the perception of pain and its symptoms are probably among ethnic minorities differ. Individuals of the Ukrainian and Vietnamese minorities hide their pain as opposed to individuals of the Roma minority. Islamists were represented by two individuals of Czech nationality, which influenced the results of the research. Furthermore, the research part deals with minorities access to pain therapy. It is important that each individual is approached individually. The results only outlined possible assumptions that would influence pain relief. Based on the results of the research was compiled information material for nurses, which briefly describes the specifics of the perception of pain in selected minorities. Its aim is to help nurses orientate in this field and introduce them pain therapy technique or to select suitable methods of its measurement.
